Waiver & Liability Release

WODE Athlete Center & Pathway Prep Waiver and Release of Liability I, the parent/guardian of the above-named student, acknowledge that participation in athletic training, games, and related activities at the WODE Athlete Center and within the Pathway Prep program involves inherent risks of physical injury. I voluntarily assume full responsibility for any and all injuries or damages that may occur as a result of participation. I release and hold harmless WODE Athlete Center, Pathway Prep, its directors, coaches, volunteers, and affiliated partners from any and all liability, claims, or demands for injuries or damages arising out of participation, including but not limited to workouts, practices, games, and travel.
